Abstract

In Indonesia there has been a demographic transition, in 1999 the number of elderly people was 7.8% of the total population, the number under 1 year old was 2.36%. In District. Gubeng in 1999 the number of elderly people was 6.2% of the total population, the number under 1 year old was 3.2%. The elderly are a high risk group, but the use of health services by the elderly is low. Mojo Health Center data in 2004, BP visits by sick elderly were 18.09%, and elderly Posyandu visits by non-sick elderly were 2.09%. The general aim of this research is to develop health services for the elderly based on the needs and expectations of the elderly as well as the provider's capabilities. This research is a cross-sectional survey research and is a case study in the working area of ​​the Mojo Health Center, Gubeng District, Surabaya City. There were 2 types of research subjects, the first subject was 371 elderly people in the working area of ​​the Mojo Health Center, taken using multistage cluster random sampling, the second subject was all Mojo Health Center medical staff (7 people). This research analyzes the needs of the elderly for promotive, preventive, curative and rehabilitative services. Analyzes the expectations of the elderly regarding how to implement promotive, preventive, curative, rehabilitative services, referrals, laboratories, schedules, appearance of officers, types of assistive equipment, social activities, and analyzes the capabilities of the Mojo Health Center . The results of this study show that the elderly's needs for health services are: counseling, regular body checks, medication and home visits for care, encouragement or exercise. The expectations of the elderly regarding health services are: group counseling with elderly health material, elderly people coming alone for regular check-ups, in the form of tablets or pills, there are home visits from officers for elderly people who cannot walk, officers wearing white uniforms, morning service schedule at 08.00, there are glasses aids, there are blood and urine tests, and there are religious, sports and recreational activities. Planning for the Community Health Center has been implemented, but sub-district level support is still needed. Implementation of planning, promotive, preventive, curative and referral services have been implemented, but it is necessary to change the curative services from free to pay according to the Puskesmas rates. Guidance Assessment has carried out reporting on the results of activities. Development of Posyandu for the elderly has not been implemented due to limited personnel, the burden of guidance on Posyandu for toddlers is very high (143 posts). There are no facilities for the elderly posyandu yet, it needs support from the Surabaya DKK through APED 11. From the results of the research and implementation of the FGD, recommendations have been prepared for the Mojo Health Center, namely: 1. Open an elderly polyclinic at the Puskesmas with a service schedule on certain days. 2. Developing 2 Posyandu for the elderly in each sub-district in the working area (two posts), with group counseling activities by cadres once a month on elderly health materials, regular examinations with KMS guidance for the elderly, treatment for sick elderly by paying according to the Puskesmas rate and tablet form of medicine or pills. Referral services, as needed, are directed to Community Health Centers, government and private hospitals, including laboratory services. Guidance of activity schedules for elderly people in the morning, 08.00-finish, third week of each month, Community Health Center officers wear white uniforms.
