Abstract
In Indonesia there has been a demographic transition, in 1999 the number of elderly people was 7.8% of the total population, the number under 1 year old was 2.36%. In District. Gubeng in 1999 the number of elderly people was 6.2% of the total population, the number under 1 year old was 3.2%. The elderly are a high risk group, but the use of health services by the elderly is low. Data from the Mojo Health Center in 2000, BP visits by sick elderly were 18.09%, and elderly Posyandu visits by non-sick elderly were 2.09%. The general aim of this research is to develop health services for the elderly based on the needs and expectations of the elderly as well as the provider's capabilities. This research is a cross sectional survey and is a case study in the working area of the Mojo Health Center, Gubeng District, Surabaya City. There were 2 types of research subjects, the first subject was 371 elderly people in the working area of the Mojo Health Center, taken using multistage cluster random sampling, the second subject was all 7 Mojo Health Center medical staff. This research analyzes the elderly's needs for promotive, preventive, curative and rehabilitative services. Analyzing the expectations of the elderly regarding how to implement promotive, preventive, curative, rehabilitative services and social activities, as well as analyzing the capabilities of the Mojo Health Center. The results of this study show that the elderly's needs for health services are: counseling, regular body checks, medication and home visits for care, encouragement or exercise. The expectations of the elderly regarding health services are: group counseling with elderly health material and the frequency is once a month, the elderly come alone for regular check-ups, in the form of tablets or pills, there are home visits from officers for elderly who cannot walk, officers wear white uniforms, morning service schedule on 08.00, there are eyeglass aids, there are blood and urine tests, and there are religious, sports and recreation activities. Planning for the Elderly Program at the Mojo Community Health Center has been implemented, but sub-district level support is still needed. Implementation of the Elderly Program Planning at the Mojo Community Health Center has implemented promotive, preventive, curative and referral services, but changes are still needed from free curative services to curative services at the Community Health Center rate. Guidance, Assessment and Development of the Elderly Program at the Mojo Community Health Center has not been able to carry out the development of the elderly posyandu due to limited staff, because the burden of guidance on the toddler posyandu is very high (143 posts). There are no facilities for the elderly Posyandu yet, it needs support from the Surabaya DKK through APBD II. The implementation of the FGD resulted in the health cadres in the working area of the Mojo Community Health Center agreeing to create a Posyandu for the Elderly in each sub-district with 2 (two) posts, promotional services will be carried out by cadres with guidance from the Community Health Center, and the cadres hope that the Community Health Center will be willing to provide preventive and curative referral services. and rehabilitative at the elderly Posyandu. From the results of the research and implementation of the FGD, recommendations were prepared for the Mojo Health Center, namely: 1. Increase the activities of the Mojo Health Center with elderly clinics, and schedule services on certain days, for example twice a week. 2. Helping health cadres to create Posyandu for the elderly in each sub-district in the working area of the Puskesmas with 2 (two) posts, with group counseling activities by cadres once a month on elderly health materials, regular checks with KMS guidance for the elderly, treatment for sick elderly by paying according to the Puskesmas rates and medication in tablet or pill form. Referral services, according to needs, are directed to Community Health Centers, government and private hospitals, including laboratory services. Fostering elderly exercise activities. Posyandu activity schedule in the morning, 08.00-finish, third week of every month, Puskesmas officers wear white uniforms.
