Abstract
In Indonesia, a demographic transition has occurred. In 1999, the number of elderly people was 7.8% of the total population, with 2.36% of those under 1 year of age. In Gubeng District, in 1999, the number of elderly people was 6.2% of the total population, with 3.2% of those under 1 year of age. The elderly are a high-risk group, but the utilization of health services by the elderly is low. Data from the Mojo Health Center in 2004 showed that BP visits by sick elderly people were 18.09%, and Posyandu visits by non-sick elderly people were 2.09%. The general objective of this study is to develop elderly health services based on the needs and expectations of the elderly and the capabilities of the providers. This study is a cross-sectional survey and is a case study in the working area of the Mojo Health Center, Gubeng District, Surabaya City. There are two types of research subjects: the first subject was 371 elderly people in the working area of the Mojo Health Center, taken by multistage cluster random sampling, and the second subject was all medical staff of the Mojo Health Center (7 people). This study analyzes the needs of the elderly for promotive, preventive, curative and rehabilitative services. Analyzes the expectations of the elderly regarding the implementation of promotive, preventive, curative, rehabilitative services, referrals, laboratories, schedules, appearance of officers, types of assistive devices, social activities, and analyzes the capabilities of the Mojo Community Health Center. The results of this study indicate that the needs of the elderly for health services are: counseling, regular physical examinations, medication and home visits for care, encouragement or training. The expectations of the elderly for health services are: group counseling with elderly health materials, the elderly come alone for regular check-ups, tablet or pill form of medication, there are home visits from officers for elderly who cannot walk, officers in white uniforms, morning service schedule at 08.00, there are glasses, there are blood and urine tests, and there are religious activities, sports, recreation. The planning of the Community Health Center has been implemented, but still requires support at the sub-district level. Implementation of the Planning, promotive, preventive, curative, referral services have been implemented, but changes are needed in the curative services that are free to be paid according to the Community Health Center tariff. Assessment Development has carried out reporting of activity results, for the Development of Posyandu for the elderly has not been able to be implemented due to limited staff, the burden of development on Posyandu for toddlers is very high (143 posts). Facilities for Posyandu for the elderly do not yet exist, need support from DKK Surabaya through APED 11. From the results of the research and the implementation of FGD, recommendations were compiled for the Mojo Health Center, namely: 1. Opening an elderly polyclinic at the Health Center with a service schedule on certain days. 2. Developing Posyandu for the elderly in each sub-district in the work area as many as 2 (two posts, with group counseling activities by cadres once a month on elderly health material, regular check-ups with KMS elderly guidance, treatment for sick elderly by paying according to the Health Center tariff and medication in tablet or pill form. Referral services, as needed, are directed to Health Centers, government and private hospitals, including laboratory services. Fostering elderly exercise activities. Posyandu activity schedule in the morning, at 08.00-finished, the third week of every month, Health Center officers in white uniforms.
